Background Checks and Conduct Review Forms:
Student Conduct provides information about your conduct history (within the last 7 years) to employers and agencies that request information. This information can only be provided with a signed release from you as the student or former student. Each agency has its own process and forms, all requests submitted to Student Conduct are completed within 2 business days (same day requests are not guaranteed). To ensure review and timely submission, please confirm that your request has the following:
Signed Release Form (signed by you, permitting us to release the information to the agency)
Verification Form provided by the agency (request for school records form, study abroad judicial review form, background check form)
Name and Address to submit the form (forms which require an official seal from the Registrar's Office can only be sent via mail; they cannot be faxed or emailed)
